Baby Dedication Form
We believe the children are a gift from God. As your church family we welcome the opportunity to journey with you as you celebrate the gift of a new life.
Baby Dedication Guidelines
1.
The parent/guardian(s) must fill out the form below and turn it into the church office at least one-month in advance to schedule a baby dedication. Baby dedications are performed at the 10:00 am worship service on Sunday.
2.
Parent(s)/Legal Guardian(s) should report to the designated area upon arrival to the church.
3.
Reserved seats will be provided for those who arrive on time for the worship service. The Administrative Assistant will notify the ushers of the names of the family members participating in the Dedication Services.
4.
Any additions or changes should be reported to the Administrative Assistant one week prior to the Service.
5.
When prompted, the baby dedicated, his/her parent(s)/legal guardian(s) and family members should be prepared to come forward.
